Provisional Membership Requirements:

Provisional (first-year) members are accepted based upon board review, date of their application, and current Active membership size.

Provisional members are expected to fulfill all of the requirements of an Active member during the first year of membership, plus participate in one additional project, as outlined below:

Volunteer with any of our sponsored organizations for at least 36 hours per League calendar year (June 1 – May 31).
Attend 5 of 9 general meetings, usually held on the evening of the third Tuesday of each month (excluding June – August).
Participate in one Provisional group service project, to be determined by the Provisional class in September.
Attend at least one summer Provisional meet-and-greet event (dates to be announced via email following submission of application form).

Membership is finalized with a first year dues payment of $125 that is submitted in August. Dues for subsequent Active and Associate years are $100 and subject to change.

Why do you want to join the Lakewood Service League? How did you hear about us?

LIABILITY WAIVER AND PHOTOGRAPHIC PERMISSION

I understand that my participation with and membership (at any level) in the Lakewood Service League (“LSL”) is voluntary. In consideration for my membership in LSL, I release and forever discharge and hold harmless LSL, its officers, directors, members, successors and assigns from any and all liability, claims and demands whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my participation in: volunteer activities (whether specifically for LSL or for an organization that LSL supports), attendance of LSL meetings and any other LSL events (collectively, the “Activities”), whether caused by negligence of LSL or its officers, directors, members, employees, or agents or otherwise. I expressly and specifically assume the risk of injury or harm from the Activities and release LSL from all liability for injury, illness, death, or property damage resulting from the Activities. I understand that this release is intended to be as broad as allowed under the laws of the State of Texas.

I grant and convey unto LSL all right, title, and interest in any and all photographic images and video or audio recordings of me made by LSL during the Activities (the “Images”), including, but not limited to, any royalties, proceeds, or other benefit derived from the Images. I understand that LSL may use the Images in all promotional material, including social media, and give my permission for such use.
